Do these things have to be from undergraduate school?

I am wondering because in highschool I did a science fair project that I took all the way to the state level. Its something that I'm proud of, and it is an Honor/Award... but is it appropriate to note for dental school apps?

I think you're supposed to put down the awards/honors you got in college. The HS stuff was supposed to be for undergrad appl.

Adding HS makes it seem like you haven't done anything cool since and are trying to make up for it. Don't use it on the app.
 
I would agree with the previous posts..to a point.

I believe that you would typically want to leave out high school information (with the possible exception of final GPA/Class Rank and/or national honors, such as National Merit Scholars). That being said, if you did something in high school that you have either continued through your college years, done at a prestigous institution or has made a lasting impact on you or your community, I would list it. If your project was one in which you followed through in college, it would not hurt your application.

A major caviat, if that is the only thing that you would list in that particular section of your application, and it only occurred in your high school years, I would leave it off. Mention it in your essay or at your interview... but you do not want to do anything that leaves the Adcoms with the impression that you did not do anything in your college years.
