Do these actually cause any real changes in the muscles/joints that help soreness or does it just create a sensation that overwhelms the pain?


I've always wondered if and how they really work.
 
If I recall correctly, they (menthol and pepper based ointments and creams) are just counter-irritants that affect the nerves closer to the top of the skin, therby "distracting" them from the pain signals being given off deeper in the joints or muscles. I think, however, that long-term use of the topical pepper products has some pain benefit...but I can't remember for sure.
 
Yup, you're right. Capzaicin has substance P in it, which comes from peppers and is effective for pain.
 
just to clarify, capsaicin which is from chili peppers and the active ingredient in Zostrix relieves pain by inhibiting substance P... as opposed to containing it, it's a chemical found in nerve endings which is involved in transmitting pain signals... and yes, most topical analgesics do act as counter-irritants
 
If I'm not mistaken (I'm not going to check google, check it yourself), many pain patches/ointments/creams are topical salicyclates (ie: aspirin through the skin).
 
ah. so the menthol never actually affects blood flow to an area?

Well, menthol constricts blood vessels, which gives that "cool" feeling.

That is true. Not long ago, I was using Flexall and it says on the box that it contains aspirin and not to use if you cannot use aspirin. I think it's a combination of the menthol (to cool and restrict bv) and the aspirin to alleviate the pain at the source.
