"For future notice the patient has a low pain tolerance." This is buried in the middle of the gibberish. Not likely to remember that one.

This "doc" would not be able to use canned notes/ shortcuts since it sounds like his procedures are a lil bit different each time.

This has to be my favorite injection note ever.
 
Now my two favs are "cutting the anesthetic in half" cause he can't see the dye.
Next fav is ending with "I did not give any sleeping pills. "

As a reviewer, I get a ton of files to go through nearly every day. But this note was actually from my fellow's pile of reviews- he was just to chickensht to post something.

Although I let him get a little liberal with the procedures I thought we were mastering- I C3,4,TON block turned into a C2 superior, waste, and C3 block.

Now he'll post....
 
I love how he throws "coumadin use" in there like a land mine.
 
i can't decide if english is this guy's second language, or if he is really that big of a dumb-ass. thats like somthing that would come out of a middle-schooler.

i agree that this is amazingly awful, but at least he's honest about what he's doing. brutally and stupidly honest...

sometimes it is tough to get the ESI and the SI and facets all at once...ugghhh.
 
So, I lost count, how many procedures did he do at once?

And for Fibro? 😱
 
My favorite part was "She was laid in the prone position." Typically one does not include sexual acts in a procedure note, but perhaps it is a cultural thing...

Thanks for the chuckle. I thinks its safe to assume English isn't this doctor's first language.

Medicine is not his first language either.

It is a disappointment to the field that folks like this are in practice.
Inappropriate to provide anything more than this: he is a Baylor PMR graduate

wow....is the baylor program that bad..that is truly an embarrassment because baylor is good at most things (went there for med school)
 
Every time I read it, I find another gem. "She got obvious sedation." Transcriptionists must have a high attrition rate.

"much of her pain is fibromyalgia"...."I will do a third epidural in a couple of months and then repeat the series"...

that should take care of the fibro
 
I have a note describing bilateral TFESI's at 3 levels for a left C7 radic. Halfway through the note it says that a Tuohy needle was inserted through the sacral hiatus and threaded cephalad.

I also have a facet injection note describing the facets being injected 3 times, 3 different ways with different needles and different volumes. It added up to about 5ccs/joint.

I have another one that says "The epidural space was searched and entered".

Doing Board reviews I see too many of these.

To be fair, one of the best pain docs I ever knew dictated the most awful op notes. In person he was intelligent and charming but on paper he sounded like the O.P.
 
i don't know if i'd want to see a doc who sounds like he is stuck in 8th grade..
 
i went to Baylor PMR too....hey wait, maybe that's my note!!!! Those transcriptionists in India are doing a great job i see. Please everyone should be doing ESI's on every patient on Coumadin. Dont bother with an INR.

where did he do fellowship? Or did he?
 
I was laughing while reading the entire note - Thanks Steve!

New procedure - not sure how he billed for it: Injection outside the facet. It must only help if you do it in conjunction with an epidural all the way to the hub 'and extra' with lots of local :laugh:

Oh dang, scary stuff happening out there

If anyone knows who he its almost a moral imperative that he be sent to some ISIS courses. They will also help you generate a good-english note

Baylor is great for SCI and inpatient TBI rehab(Congresswoman who was shot is there i think), but i dont think they are particularly known for their pain program. Its been about 9 years since i was there so maybe im wrong. I dont keep in touch with them.
 
Thanks for the post. I am starting my practice soon and was looking for a good template. Now I have one template to cover several procedures. It's perfect!! Thanks, SDN!!!!
