There are what maybe 50 spots a year. We have very few attendings who post here. The chances that one of them did a FPRS fellowship is pretty slim. If you decide to do one of these your faculty FPRS person will be able to help you navigate the applications process. Plus, http://www.aafprs.org/ has a list of all the fellowships and what they mainly do and offer to you as a trainee.
Had a friend who went to Toriumi's fellowship in Chicago. Very good for cosmetic and overall. Doesn't prepare you for complex recon (free flaps)
Overall, most FPRS fellowships are usually tilted towards cosmetic procedures (rhytidectomy, rhinoplasty, brow, blephs, etc) are less towards more complicated recon. Then there are head and neck cancer/microvascular fellowships which lean way towards recon and much less towards cosmetics.
Really, the jobs are so different. Cosmetic guys usually have fancy offices tilted toward private patients who want "special" attention. Microvascular guys are almost always in big academic centers and see head and neck cancer patients (not your typical cosmetic patient).
