best time for subspecialty elective rotations

Hi
I am an intern from a univ affiiated small community program in nj. I wish to apply for fellowships on h1 for 2008. What is the ideal time to do electives for the same in the pgy2 year, during or prior to the interview season?
also I want to know if rheumatology and endocrinology are availaible for h1 visa status.
