How does beta blocker work in heart failure? The heart tries so hard to compensate by increasing its sympathetic activity and why do we want to block it anyway? Is decreasing work load is all that ? it is still confusing? We even give dopamine sympathomimetics..........so why give beta blockers in first place? Can anybody help?

This is why, for many years, cardiologists didn't use BBs in the treatment of heart failure. Simply, continuous activation of SNS (as seen in heart failure) will cause myocardial damage, and the therapeutic value of BBs is due the inhibition of this deleterious effect.
