Book/reference for preparing treatment plan?

Our school wants us to prepare a treatment plan for every patient we workup. Is there a good book out there that says what exactly you do (i.e. what drug and in what amount/what test(s)) for things like ADHF, acute pancreatitis, alcohol withdrawal, etc.?

Seriously though, med students usually use whatever pocket book is appropriate for the specific clerkship, like Pocket Medicine or the Mont Reid handbook or whatever, to help with treatment plans.
