I cant believe someone can work 20hrs a week and get paid 110k thats great. I wonder if psychiatrists can make millions and how that would happen.

There are psychiatrists that charge well over 500 per hour and are booked for more than 40 hours per week.
Forensics and sleep can also make this.

The psychiatrist I am personally acquainted with who got quite wealthy from his work did this primarily by opening free-standing inpatient detoxes that he then sold to a larger group. Note that this requires several skills that are not, as far as I can tell, part of the curriculum of any psychiatry residency.
