any recommendations as to where a licensed physician who has finished residency can get good training in botox?

The petty side of me would say:

yeah, it's called dermatology / plastic surgery / ophtho residency...

but, I hate Botox, so best of luck with the Botox rep. Be careful not to drop anybody's brow -- I hate having them come into the office wanting to know what can be done to correct it.

and yes, that does happen once or twice a month (and I do not practice in a cosmetic hotbed area)
