so, i figured out that if you spend your whole month on pulm critical care and every time you are asked to analyze a chest x-ray and you answer either sarcoidosis or wegener’s, you will not pass the rotation.

an example:

75 yo male with extensive smoking history presents with unremitting cough, hemoptysis, night sweats, and recent weight loss. Chest x-ray reveals a large cavitating nodule in the right middle lobe. Your differential sore eye? Well, mr. attending, this is a clear cut case of either sarcoidosis or wegener’s. I say we hit him hard with steroids and send him home.

another example:

14 yo male with multiple sinopulmonary infections throughout his life shows up with hemoptysis, fever, and white count 22K. Chest x-ray shows widespread bronchiectasis and situs inversus totalis. Your diagnosis sore eye? Dearest attending, this appears to be a very severe case of sarcoidosis occurring most prominently in the left lung. In fact, the sarcoidosis is so bad in the left lung, it appears to have scared the heart into the right side of the thorax for fear it might be consumed in a granulomatous calcific haze of sadness.
