CA Law question on Dosage Formulation Switch Please

Hey fellow CA RPHs, applicants, any one that knows this answer pertaining to CA LAW:

1) Question on ability to switch dosage formulations on a prescription (page 23 of CPJE book)
- Can we automatically switch a drug's dosage form on a prescription WITHOUT the doctor's approval as long as the drugs are the same and for the same treatment as long as it's in the benefit of the patient?
& if you can switch, do we have to contact the prescriber 1st?

- It says you can switch from tablet to oral solution if you know it'll benefit the patient (adherence in this case).
- Can you switch from tablet to capsule?
- Can you switch from ointment to cream and vice versa? --> I'm assuming NO because they are different potencies, but I'm double checking...

Any help is appreciated!! Thank you! I think this is a new law update but I'm not sure..
 
Yes, you can. As long as they are bioequivalent. Ointment and creams are not bioequivalent. Ointments are more potent.
