Hi I am a foreign student .Can any one please tell me the requierments to appear for the exam other than passing part 2 ?.Where can I find the application forms for this exam? is there any application forms available online .Whom I should contact for application forms the state board or ADA .How helpful is Dr Duggans study institute to the exam prep? Any help is greatly appreciated .Thanks
