I was in the military for a few years and attended college part-time. During that time I worked my way up to shift leader. I was responsible for a family practice clinic containing 4 doctors, 2 nurses, 6 med techs, and 2 admin staff. I was also the logistics manager for four family practice clinics and an internal medicine clinic, overseeing a budget of millions. This was by far the most significant leadership I have had. The only other leadership would be as an officer of the hospitals booster club; this was a volunteer extracurricular activity I did outside of my regular military duties.

Can I count either of these as leadership or would it be "double dipping" seeing as though it'll be under my paid employment? I don't want it to seem as though I'm fluffing up my application; yet at the same time, I don't want it to look like I have no leadership experience, when I actually have a significant deal more than most applicants.

Dude, that's awesome, and way more significant than most student "leadership" (ie, I founded a journal-reading club that five other people attended for a few months until we got bored and drifted away).

Yes count it, and make it one of your "most significant" activities so you can expound upon your role.

Definitely count it! I don't consider it "double-dipping"; the best leadership positions tend to come from employment.

that's definitely leadership, just mention in the description that you were paid. oh, and that's awesome!

Count it and make it one of your most significant activities. When you expand on it, admissions committees will see that you are definitely not "fluffing" your application.

Either could be listed as Leadership. So long as you don't state the hours of involvement twice, you won't be "double-dipping." Many exmilitary divide out their time in the service into different categories so as to have enough space to explain everything.

Alternatively, if the clinic responsibilities were the primary thing you did in the military, you could still list it under Employment-Military, but include something in the title of the activity (as well as in the description) that indicates the leadership component.

Or, if the description of the experience of being in the military already used up available space, list it as Leadership, but don't fill in hours per week in the header for one of them.

Other strategies could work as well.
