Can I manage to skip January interviews?

I think you're making this a lot harder than it needs to be.

Say yes if you really want to do this. Your interviews will sort themselves out over the next 6 weeks or so. Maybe you get your IV offers from the "top" places (which, for the record, you may hate) that go into January. If you do, and you absolutely want to go there, don't get on that Delhi-bound plane. I seriously doubt that your presence or absence at this WHO thing is going to have any meaningful impact on anyone other than you.

FWIW, I interviewed well into January (11 years ago, also from a crap school, with mediocre scores, great grades, AOA and a PhD). Other friends were done before Hannukah.

Ha, funny that I'm considering something similar, albeit for some leisure travel during January.

I'm doing what you've proposed to do: wait it out and see if you get any interviews at top places, even if that means waiting 3-7 weeks. I know it sucks and you'll end up spending more money for your flight. Or if your WHO colleague sets a hard deadline for you to decide. But it's good for peace of mind. It's all about risk reward. Who knows, maybe you get an early January invite for UCSF and that's somewhere you really want to go. That's very high risk. Or you get an MGH interview but maybe that's not the best fit for YOU, even though it's #1 or 2 in the country.

If you applied to enough programs and are even slightly competitive, you should be able to fill up your interview season with October and November interviews alone. I had 17 or 18 offers by the beginning of October and I could have gotten to all of them within 8-10 weeks except I was on two audition rotations until December. So my December and January were really busy. :-/
 
My IM residency director said that I could fit in all my interviews in Oct/Nov/Dec.

He's right.

Unlike most other specialties, your typical IM program has anywhere from 20 to 50 interview dates scattered across the season. You are an excellent candidate and will be able to schedule them earlier just fine.
