Perhaps I am a bit confused about your timeline. In
this prior thread, you went into some detail about your past. I had thought that you:
1. Graduated from a Carib school in 2004 or so.
2. Scrambled into a PGY-1 spot in IM.
3. Trained for about 10 months, left after being threatened with probation (or something similar).
4. Did some time in a nonACGME approved surgical position.
5. in 5/08 had "...tried the last few matches and got 1-2 interviews, last year nothing from the NRMP match, but never got back in."
Although there isn't a clear timeline layed out, it would seem that you are at least 5-7 years out from your medical school graduation. You did some training in an ACGME residency program, but I had the sense that this was some time ago. My apologies if I have read the situation incorrectly.
To answer your question: I'm not sure what you mean by "qualify" for a PGY-1. As an MD grad, you can certainly work as a PGY-1. However, your distant graduation, low USMLE scores, being dropped from a residency program, and lack of support from those you have worked with is all going to work against you. It should be clear by now that continuously reapplying via the NRMP match is not a working strategy, assuming you have applied broadly to (relatively) non-competitive FP (and perhaps psych) programs. Your best chance of success is via the people you have worked with. You mentioned that you worked in a non-ACGME surgical spot. If the people in that program really thought you were good, they would find a way to get you a spot -- inside or outside the next match. If they don't, then they think that their lowest match off their rank list is better than you, and that's very telling. However, that might be because the program is "prestigious" (your term, I believe) and hence does not take IMG's or those with low USMLE scores, no matter how hard a worker you were. In that case, I would hope that they would help you by putting in a good word with other programs. If not, that also is telling.
Bottom line is that your chances of getting another spot are small and shrinking quickly. A new PGY-1 is what you should be looking for. IMHO you are wasting your time looking for a PGY-2. Networking with those that have worked with you is your best chance.
