Are we supposed to be clicking the certify button or whatever every 3 months that our practice setting hasnt changed? Is this mandatory even if our practice is the same?

CAQH at least is reasonable to work with and update. Few gripes.

if our practice info stays the same is it a big deal if we dont go in and keep attesting every 90 days?

No idea. Never done it. I know CMS did something to force this issue. So I've been proactive not wanting to rock the apple cart. It's annoying, and makes me want your name. But then I think back to working at Big Box shops and I remember this is still better. Oh, and the Big Box Shop in the past would still have me do it anyways...
