Which one usually comes first? Carms or NRMP? is there any way of knowing in advance? It can make a big difference for those wanting to come back to canada after getting a US MD

Excuse my ignorance but when you rank your programs do you have to rank every place you interviewed at?
Let's say I interviewed in few general surgery and one internal medicine in Canada and then I ranked my choices and eventually decided that if I don't get gen surg in Canada I will want to try for it in the states (Given that I have interviewed there already...)
How should I work it out?

ROLs only include programs that YOU WANT TO BE RANKED. You don't have to put places that you interviewed at and, likewise, you can rank places that you actually never interviewed at (but that would be silly 😉 )
With your example above - don't apply to FM if you desire to do surgery in the US or Canada if you want to play it safe. I actually don't know what would happen to you if you got into a FM program in Canada and tried to transfer out the next year into a surgical program. Many slots for IMGs have it tough in doing so as the spots they match into are the IMG slots which mean that there is alternative funding -?from an IMG money pool - for you as the resident and that I believe comes with strings (unlike the Canadian residency spots) in dealing with transfers to other programs let alone other provinces.
I understand that you'e already applied both to CaRMS and NRMP/ERAS.
I am an IMG, live in Canada, and plan to do the same this year. What I don't quite understand is the statement
on http://www.carms.ca/eng/ERAS_intro_e.shtml:
'ERAS. CaRMS acts as the Dean's Office (DWS) for Canadian medical students and graduates ONLY applying to positions in the United States.'
What about the IMGs? I suppose that we will have to use both NRMP and ERAS when applying for the US match and CaRMS for applying for the Canadian match.
Is there a cooperation between CaRMS and ERAS for IMGs? (like CaRMS sending electronic copies of the received documents to ERAS or so?)
Then, it seems that the deadline for submitting the LoR to ERAS is 1. August (http://www.ecfmg.org/eras/2012-timeline.html), vs. mid November for CaRMS. I find it a little bit strange, as the references for electives/observerships done after July won't be taken into account.
Is it right?
Please tell me about your US and Canada application experience, any tips?
