Case-based Internal Medicine book (not for shelf)

MS4 here. I'm not talking about books for shelf exam. Just want one that's case-based with some questions to stimulate thinking and learning around a topic cuz I find it sticks better when it's presented as a case... I already have Case Files... Can anyone recommend one that fits the description?
