There was JUST a thread where someone mentioned that they had failed an early exam (or series of exams) but found that they learned best through clinical cases/vignettes and subsequently blew up step 1 (260+ is my recollection).

Does anyone have a good resource for this type of learner?

Specifically, I'm sweating basic (foundational) subjects: biochem, embrology, genetics, anatomy, etc.

I'm sure I can hunt down organ-based stuff.
 
The best thing is a qbank like USMLE-Rx. Most questions are vignettes, and you can separate them based subject. After every question they go over each answer choice and tell you why it's right or wrong. USMLE World is also a possibility, but might be a bit much for the first year.
Edit: This won't be enough for your school exams. They will test on you details that have nothing to do with clinical medicine and won't be in any Step 1 review source.

First Aid Cases is a great book. Presents vignettes and then asks questions (not multiple choice, just questions about etiology, pathophys, mechanism, symptoms, treatments, etc)

Yes another excellent choice to get used to thinking of concepts as clinical cases.
