Case logs for hospital credentialing

Hey everyone, new on the forum. I am starting a new job and have been asked to supply my last 24 months case logs for review and they ask that it be HIPAA compliant. PLS has a very neat PDF print view of all the logs, but unable to sort through dates and the last name and birth year of patient is listed. The excel file is obviously filterable but when I try to print to PDF it is completely disjointed and I'm unable to figure out how to make it look neat. Anyone run into this before? Any problems with submitting the PDF of the logs that gets printed off of PLS including the patients last name and birth year? Thanks

I am not sure they really want to see all 24 months of cases with patient info.
I vaguely remembered something similar when I first joined the medical staff. All I did was to print out a PDF summary page with the total number of procedures and type of procedures (forefoot, rearfoot, trauma) performed during the 3 years of residency. I believe there are 2 of them: Clinical Log Summary and Procedures Summary. That's all I gave them and they didn't ask for anything else.
