Cash Procedures at Non-Profit = Holy Grail?

Any experience doing cash procedures at a tax-exempt charity hospital and having the patient and hospital characterize the financial transaction as a "donation" and not a professional service? The patient gets a tax write-off, the hospital receives a tax-free donation, and the doctor gets a piece of the action. Win-win-win.
 
I don't think legally you can write off the cost of a service, even to a non-profit. Obviously these things happen on a small scale - I'll make a donation if you give me xyz - but institutionally, this would be fraud I believe.
