Categorical vs. Advanced on ERAS

I am applying to anesthesia and I have a few questions. I have searched the web and cannot seem to find any good sources to answer my questions. If you know of a good resource for the answers to my questions that would be great.

1. When applying to both categorical and advanced programs at a particular institution, do you have to pay 2 separate fees? Additionally, is there a separate fee if you apply to preliminary program at an institution that you are already applying to their advanced program?

2. How do you know if you are interviewing for advanced vs. categorical or both? Once you get offered the interview does the program let you know?

3. How do you select preliminary (medicine) programs in ERAS? There is not an option to select 'preliminary programs' the way there is for specific programs like lets say 'anesthesia' and so on.

Usually both. I presume if a program thinks differently, they'll tell you. You should probably still rank both regardless.

Apply to Internal Medicine programs and select the box that says Prelim. Just like you select the box that says categorical vs advanced when you apply anethesia.
