Any one got beef with patients using CBD during treatments? Anyone encourage it? Been getting asked a lot by patients. I usually tell them it's known to help with nausea, seizures, pain, but don't expect it to kill the cancer. Certainly wasn't exposed to this in med school, so wondering what other people's thoughts are. Have even seen some doctors in my area sell CBD oil in their office...(not rad onc).

I tried it, felt nothing and then downed the whole 60$ vial, but still did not feel anything. Dont have a problem with patients using it.
I think it needs to have THC in it for a real benefit, and THC has to be decarboxylated/heated to activate it?

If you were looking to get high, you should know what to do. CBD aint it chief

Is it at all regulated and/or anybody know the reputable companies or brands?

It seems like entire stores that sell nothing but this stuff have popped up on every corner but now I even see them at gas station counters next to all kinds of crazy stuff . . . not the least of which are vitamins that claim all kinds of benefits but looks like a random collection of pills taken from a medicine counter and thrown together in an old ziplock bag with some a horse would have difficulty swallowing!

Multiple cannibinoids in marijuana, only one of which is thc, they all have different effects, unfortunately the right wing agenda has never been pro science hence MJ remains a schedule I at the federal level despite clear clinical and preclinical evidence for medical utility
 
No problem with people taking CBD or MJ. I can't exactly encourage MJ use in my state, but I certainly don't tell them not to. Biggest problems facing many of my patients; pain, anxiety, weight loss, nausea.... line up really well with MJ "benefits". I think there is randomized data out of Canada showing better treatment tolerance in H&N patients with THC.

Can't patent a plant that's been around forever. Pharma lobby is strong.

No beef. I encourage it for patients whose main issue is appetite stimulation, as I have anecdotally seen it work better than Megace or 'pharma' versions.
I don't 'recommend' it for nausea or pain, but if patients are taking it I don't tell them to stop. I don't prescribe it, most people say they have a means of accessing it. Encourage use of CBD oil or edibles rather than anything involving smoking (including vaping).

Discuss that it won't kill the cancer. Haven't used or endorsed it for seizures either.

Did you have nausea or pain or low appetite before hand? CBD's whole thing is that it doesn't get you high reportedly. Although if you didn't get the munchies afterwards then maybe it really did nothing.

No, just wanted to see what it felt like, maybe help me sleep. May have been bad batch. This stuff really needs to be regulated.
 
There's one FDA approved CBD oil based drug, the anti-epileptic Epidiolox. In the 2020 budget, there's finally money for the FDA to study/evaluate Hemp derived CBD oils.

How did the FDA approve something derived from a schedule I drug (aka "no known medicinal value")?

Still doesn't address the enigma of how marijuana remains a schedule I drug despite derivatives from it clearly showing the medicinal value of that plant

This actually came up today with a patient. I didn't realize until I looked into it that its an anti-oxidant. Its probably fine but I discuss with my patients the mechanism of RT and tell them I would personally avoid it.

I have discussed this with a medical oncologist at Hopkins and there appears to be immune suppressing effects of CBD/THC. He did not have any references but I didn't ask for any. It may have been some lab research but as I recall they were seeing decreased efficacy of immunotherapy.

N=1 so take it for what it is. I treated someone (a friend of the family of course) for a nasty esophageal cancer a couple years back. He struggled with gastroparesis and started on CBD with good results. Still taking them. Was started on Pembro (no chemo) for peritoneal recurrence about a year ago and is still NED (as of imaging after cycle 3).
 
The way things are looking perhaps everyone will be in the MJ game including the russians/chinese, John Boehner, and many rad oncs. When the breadline comes can pull together brains and start a clinic somewhere or a dispensary. Gotta put money on table somehow!!
