I received a booklet from one of the top Caribbean Schools In the back they list about 1840 post graduate residency appointments (names of individuals and hospitals) for 2011. these all cant be from 1 graduating class. not even 2 classes. How did this number get so high?

Multiple reasons. At some offshore schools, you aren't allowed to sit for USMLE until you meet certain internal yardsticks, so the folks who start together dont always finish in the same year. Second, because rotations are hosted by stateside hospitals which can cater to multiple schools, enrollment is limited, and sometimes offshore folks have to wait an extra year to get into a core rotation they need. Third, the classes are very very big to start with. Fourth, the USMLE pass rate isn't amazing, and so some people are going to be spending time to study longer to take/retake it. And finally, sometimes coming from offshore schools you are forced to do the match multiple times before you end up in something categorical. The short answer is that unlike their US counterparts, for a lot of offshore students med school isn't just a four year adventure, and since the timing is variable, its not uncommon to have a lot if people in limbo for a year or two, with many applying for residency in the same year.
