chances for top programs

I'll be applying next year in Anesthesia, but as I start to think about away rotations and fourth year - I was curious what sort of chance I might have at some of the top programs.

I'm interested in academic programs, as I am hoping to do critical care afterwards also.

240/99 Step 1, great leadership, some unique EC's, should have some case reports (nothing published yet), should have some great letters, good clinical grades, shooting for a good Step 2. Lower tier, allopathic school.

Would I ever have a chance at Stanford, BID/MGH/Brig, Mayo etc. ?

What programs should I consider? No regional preferences.

You might as well just apply to all of them because it's easy, you'll probably get some bites but if you really want to help yourself match at a top program you'll need to do well on an away. Nothing really stands out on your app now, 240 is alright but scores are rising fast and it looks to be around 75th percentile now
 
You will get interviews at probably all the top programs. You do not need to worry. I would suggest looking at NRMP Charting Outcomes of the Match 2011. That can answer many of your questions.
