Define Mid-tier? I don't think you are remotely competitive for mid-tier university programs. You should be able to match a community program though without much issue. Hard to answer your other questions without more specifics. Do some audition rotations at local programs, perform well on them. Get good LORs.
 
Just apply broad, be reasonable and take everything you hear from people on SDN with a grain of salt. There are large and small community programs out there. A lot of these programs have websites that are easily searchable. Concentrate on doing better on COMLEX 2, that will help your application a lot.
 
By mid-tier I was thinking something that was a decent DO IM program or an average IM program, I don't know what community program exactly entails vs academic from what I searched up academic is associated with a university or hospital whereas community is smaller?
Yeah basically. University programs are more prestigious, usually have more fellowship and research opportunities.

You aren't competitive for any program that starts with "University of" But you will definitely have a good shot at any community program.
