Hi

I need some advice.I joined as a categorical medicine resident now doing PGY1 .I was never really interested in doing H&P,but thought I could manage to do it and do a good job.but well into 1 week of internship, i feel i am not doing justice to patients coz i am not taking proper H&Ps since I just dont like to do it.

this thought of doing some non clinical field did come to my mind before and I thought of doing some branch other than int.medicine ,but I didnt know what to do.I didnt like patho coz it involves autopsies which i really dont like.I of course wouldnt mind doing radiology,but as an IMG with no research ,scores in low 90s,no USCE I cant dream of it.


1.My question is if i leave categorical med,at the end of PGY1 is it credited as a 1 year GME ?

2.is the syllabus for categorical and preliminary medicine different?

3.Do you think doing nuclear medicine after 1 year of medicine is a good idea?
what other non clinical field can you recommend?


what is the scope of nuc med?

I definitely wouldn't do nucs without completing some other residency 1st. Physicians trained in nucs and nothing else have a hard time finding a job b/c most practices (even in academia) require their nucs docs to do other things. The ones trained in radiology read other studies. The ones trained in medicine (usually cardiology) see patients or read other studies like ECHOs and EKGs. I think you may just be a little shell-shocked from your first week. Give it some time and see if you change your mind. If not, I'd look at switching into another residency program before doing a nucs fellowship. Just MHO. 🙂

Nuclear med is a therapeutic as well as diagnostic specialty. You need to be able to get a good focused h&p if you're planning to obliterate someone's thyroid. History is everything in most specialties so it kinda is the center of the turf. I agree..give it some time. It'll get faster, easier and more focused. If you really don't like doing H&Ps, consider pediatric oncology. They usually come in with no prior medical history except for what you are starting for the next poor beggar to pick up the patient. ( 😉 😎 🙄
