Hello all,

I am trying to put together a survey for mothers and mothers-to-be about childhood immunizations for a community health project my school requires. The survery is trying to acertain common myths and misconceptions parents my have concerning vaccines, what prevents some mothers not wanting to vaccinate.. As a 3rd student, I am having trouble thinking of enough questions on my survery and wanted to see if you all had any suggestions.
