What should you do if being an anesthesiologist means more than the location your at? What I mean is should rank your number 1 at a place you think you have a better shot of being accepted or risk not matching?

That is not how the match works. You rank your preference where you would like to go, and the programs rank their applicants in their preference.

You ranking a place higher doesn't move you up the programs rank list. You could rank 10 derm programs you didn't even interview at before any anesthesia programs and it wouldn't affect your chances.

To add to that, the optimal rank list strategy is always to rank in order of preference.
