Are you interested in doing an elective as a medical student in the US or doing your residency in the US?
If its the former, there is no centralized service but in general you have to have:
evidence that you are in your final year of medical school
that you are in good standing in your medical school
you ahve health insurance good in the US
a passing score on USMLE Step 1
appropriate visa to enter the US
Some programs require that you find your own malpractice insurance in which case you are probably out of luck...no one here has ever been able to find independent insurance which is affordable. You apply to programs individually; most have information on what documents they require (in addition to the above) on their web sites.
If you are talking about a residency in the US (ie, post graduate training), then go to http://www.ecmfg.org for all the details about what you need to do.

Thank you so much for that information...i indeed want do electives in any institute,but I havent written my usmle,so i doubt,if they would accept my application and could pls tell me how much does the institute charge for this and is there any wavier for international students!!!

Some places, Harvard and Stanford come to mind, charge the equivalent of 1 month's tuition...this can be upwards of $3,000 USD. Many, many places charge nothing. Some may charge a couple of hundred dollars...it really varies.
IMHO, foreign students make the mistake that it only "counts" if they do an elective clerkship at some internationally known institution. Perhaps, but consider the fact that these places are rife with clerks and you might find it more difficult to get the attention you need to get a great LOR. Besides, if you have financial difficulties it seems a bit ridiculous to spend $3000.
I am unaware of any waivers for foreign students.
Here is what you need to do:

You will not find a program who will accept you for a rotation without a Step 1 score. All US students have to pass it to do their clerkships so foreign grads must as well.
