Well by the time you're ready to apply for fellowships, things could be different. It's possible that more of the fellowships will go to a match system (currently only peds anesthesia has one). And depending on what happens with the job market, fellowships could get more (or less) competitive. I would guess more.
There are only a few things fellowships can look at. There are no "grades" in residency. You have ITE scores, LORs, strength of residency, and research. That gets you an interview. Fellowship applications may be due before chief resident appointments so you may or may not have that.
