Comprehensive list of questions for the dentist LOR

I've been searching these threads and I don't think anybody has asked these questions regarding the dentist LOR so here goes...

Is there a certain letter of recommendation template for the dentist to follow that is writing me the LOR? Should it look like this (http://jobsearchtech.about.com/od/letters/l/aa021400_ex3.htm)
where the contact information for my dentist is included at the top, the date (which I'm guessing is when he completed writing the LOR?) , dentist signature at the bottom, etc.?

Also, I plan on applying to 10 schools and having myself send my dentists' LOR to each school directly. So do I tell my dentist to make 10 copies of his LOR and have him sign each one or have him sign one and make copies of that? And the dentist needs to sign the flap of the envelope for each school I'm applying to? Lastly the mailing address for the dentist LOR can be found at https://aadsas.adea.org/ under the tab Participating Dental Schools? So if I were to send the dentist LOR to University of Alabama, the mailing address for the dentist LOR would be

University of Alabama at Brigmingham
School of Dentistry
1530 3rd Avenue South, SDB 125
Birmingham, AL 35294-0007

Also, should the dentist LOR include my AADSAS ID number or put the number on a separate piece of paper?

u send ur lors only to aadsas. just one copy of them. doesnt matter how many schools ure applying to. and u send it to aadsas with the lor matching form thats on the application. ppl, read the instructions. seriously.

But I'm not planning to send my LORS to AADSAS...I heard AADSAS takes a long time processing and sending them out and I heard its much quicker to just send them directly to the schools...

You should make sure that the schools you are applying to will let you send the LORs directly to them.
