Conditional Probation vs. Probation

I have noticed that on virtually every med school application that we come across, they ask for disciplinary status. I have been placed on conditional probation at one point (something that isn't on my record, but is more of warning). Should this be reported if the question asks whether we have been on probation?

Probably a tough question, but any opinions? Thanks!

It sounds to me like a form of probation. If in doubt, create a different email account, and ask the medical school(s) in question.
