Confused about LORs, please help!

I'm sorry, but I have a quick question about LOR. Is LOR the same thing as evaluation form, or is it totally different? Are the professors supposed to actually write a separate letter AND fill out the form? Am I confusing myself? Please help, I am freaking myself out and it's not even end of April yet =( Thanks
 
LOR stands for Letter Of Recommendation, so yes the professor will have to write a separate letter.

I hated that. Why two different forms. I know my professors get tired of me asking them to write some any things for me.
 
I have been searching about these LORs stuff but still dont understand. Could someone please help me out with these questions:

1. Who should the letter address to? Such as "dear admission committee" and what address should it be sent to?

2. The other form that the professor has to fill out, can we get it off from somewhere or we have to wait until mid May?
