Confusing application fields

For UOP's online application, they have 2 different fields for extracurricular activities.

Activies Please list your activities (in or out of school) in order of their importance to you (this includes sports, service projects, clubs, special interests, and programs such as MESA, College Summit, AVID, Upward Bound, Talent Search or Gear Up).
The form looks like: Activity, Position, [Check boxes for 9th, 10th, 11th, 12th]

Work Experience Please list principal jobs, including summer employment, you have held during the past three years.
The form looks like: Position, Employer, From Mo/Year, To Mo/Year, Hours per week.

Where do I put volunteer work and hours? What about a job-shadowing experience?

Yes, I did call the admissions office. She told me to put volunteer work in my personal statement area or work experience (the tone of her voice sounded uneasy, which makes me feel uneasy).
- I'm putting my essay in the personal statement area and so that idea is out - unless I can press ENTER a few times and put extra information, haha
- If I were to put the volunteer work under 'Work Experience': I want to list the total amount of volunteer hours I have. The 'Hours per week' won't fit since I volunteer every other week.
- Job-shadowing was just ONE day so it wouldn't fit there either.

Do what the lady told you lol. Volunteering is an activity though but I don't really know. Job shadowing is activity, and I don't think personal statement goes under either.

Should I put a brief description of each activity? Saying that I was a member of a club doesn't really say anything about what we do.. UC apps had a space for description, but UOP's doesn't: only activity and position.
 
Are you applying to University of Pacific (UOP) in Stockton,CA?!

lol I'm applying there also! (Well, I have already applied and submitted my app, scores and letter of rec)

Are you applying for the pre-dental program there? I heard it's really good. They also have a really good pre-pharmacy program.

How did you fill out the activities part? Did you describe the activity or just put the position, like 'Member' or 'Officer'?

For the online application, they only want you to list the activity, school year(s) and your position held (i.e. secretary or member etc). No need to describe the activity.

I sent my transcript and letter of rec already, I just need to submit the actual application. Do I get sent the ID # once I submit it? I would think UOP would give me one already since the other parts of the app are in (or at least, I hope they got there safely. It's been a week or so now).

I received an email from the admission's office (after submitting app) that said they would send a #ID through mail, after they process the application. So, I'm assuming you will not get you ID till they've processed your application. Though, I suggest calling the school and possibly asking for your ID.

I submitted my app on OCT 29th, and I haven't received anything so far.

I see, thanks again! Do you know if SAT II: Chemistry was absolutely required? The website said we should have taken it, but I chose to take SAT II exams in Math IIC and Biology. Hopefully, they'll have a chemistry placement test for those who didn't.

Should I mark that I have taken the SAT exam and input my best scores? Or wait til I'm done with Dec's SAT exam (so I'd input the planned date instead of giving any scores)?

I was told NOT required but, recommended. I read on the website it's only recommended for the the accelerated pre-dental, pre-pharmacy programs and science majors. However, If you don't take it you can take their placement test. Here is link: http://web.pacific.edu/x5023.xml I suggest you put you best scores so far and; input that you plan to take another SAT in Dec.
