Hi everyone,

The ASC I do most of my procedures at is requiring me to use (and provide) updated consent forms that are unique for individual procedures. We have been using generic consent forms (bleeding, infection, damage to nearby structures, etc) rather than procedure specific consents.

Is there a good source that I can find consent forms for the bread and butter procedures in addition to advanced procedures (scs/drg trial, scs/drg implant, si joint fusion, sprint/nalu PNS, kyphoplasty)?

I appreciate any direction for this. Thank you

Can you tell them they need to come up with out since they are the bozos that require it. I’ve been using a generic consent at my hospital for 8 yrs and the hospital actually refused to make me individual consents when I asked. So apparently it’s not required
