Contact information in AAMC work/ activities

Hi,

I have 2 questions about the contacts I must include for each work/ activity in AAMC application.

1. One of the activities I included getting into the president's list of my college. The award was sent to me from the office of the registrar. There wasn't any contact infirmation other that the office of registrar. Is it OK to include the information of an office not a person?

2. Other activity I did organizing a booth in a local festival for elementary school kids. It wasn't a part of an organization. I just volunteered to do it myself. What should I put in the organization name? How about the contact information?

1) Yes.

2) Use the name of the school and whomever gave you permission to set up the booth, or yourself.
