Hey guys.

I'm trying to fish for ideas doing some self-employed business on the side.

FIRST IDEA:
-Get to know case managers at hospitals as a Vendor
-My service will be MTM in the big disease states, HTN, Lipids, Diabetes, COPD. I will counsel the patient day 1 after discharge, follow up in 15 days and again in 30 days. My parameters for success is medication adherence and what % of patients had a readmission in 30 days.
-Since hospital are pressured to stop readmissions w/i 30 days by the government, they are looking for different strategies.

Questions: Do you think hospitals will pay for this service for uninsured patients or should I hunt down patients with health insurance plans that pay for MTM?

SECOND IDEA
-Become vendor for PBM
-Offer to liaise with prescribers to maximize formulary adherence.
-My angle is that a clinician would provide better results than an adjuster


These are just random ideas i'm thinking up while bored on rotations. Maybe I should just jump on a staffing company like everybody else for some extra dough. Blah.
 
...a PRN hospital or retail job is the way to go!

We have nursing-unit based pharmacists at our hospital who's job is MTM/counseling-esq to prevent readmissions.

Who pays for it? I don't think anyone doubts the value of these services. Getting them paid for is another issue.
 
That's the main roadblock. We're dealing with soft dollar savings here.

The marketing angle is to help lower BP/Sugar/Lipids/Asthma attacks. But is willing to pay for medical advice.

Do I got to group health insurers and say "Hey, pay me to do what the retail guys are supposed to....Let's be real, we know none of them are counseling anyways and paying me guarantees that the patients are going get the right advice. You gotta do something to curtail these costs, so let's do this and do a prospective study while we're at it"

Do I go to worker's comp ppl and say, "hey let me actually talk to docs about this tylenol thing, you know they're just gonna throw away your letters. Let me talk to the patients about long term nsaids and GI protectants....now pay me $$$?"

Did I read that you work in Louisiana. Im a P2 and I might be moving there from another state. Will I be able to get a hospital job out of school in LA (without a residency)? Can it be done? How is the job market there? Lafayette. Hospital PRN jobs. Any feedback would be greatly appreciated, like companies to work for, or companies to avoid.
 
I did not mean to steer this thread...

How about offering warfarin monitoring or expanded vaccination services, such as vaccines for travelers.

I've never lived in Lafayette, so I don't know very much about that particular market. I've worked in the area a few times, but that's about it.

If you have any questions about the rest of I-10, then I'm all ears. 🙂
 
May have better luck offering those services to a private physician practice, along with consultant pharmacist services to make sure they're compliant on all the typical regulations.
 
How about the Lake Charles or Baton Rouge markets. Are there jobs to be had there.

The hospital side in Lake Charles is very saturated right now. It's fairly easy to get in whenever there's a position available, but there never seems to be anything open. Baton Rouge has a couple of PRN positions open right now, and in the past, pharmacists were able to work there with just retail experience.
 
Good to hear from you, thank you! Are you referring to Ocshner? What are some of the hospitals that I should look into?
