What is the best code for CV joint? I used intercostal block with 77002. Coding dept at hosp had no clue but thought that 'sounded fine'. I was already doing two thoracic facets in the same case so did not want to use that code.

77002 or 77003? I would assume the former. Do you have a source/citation for this (small joint)? Diffculty / risk much more in line with intercostal IMO (pneumo town). Seems like a rip off to do small joint, but I can see that being 'legally' appropriate, if not conservative.

Any other opinions?

I was doing it for painless weakness :laugh:, just kidding (reference to 'cesi' thread.)

I was 3rd opinion on a LOL (actually obese LOL) with chronic L sided mid-lower TS and "rib pain". Some pain even works into LUQ. Approx T8-T10ish area. MRI showed disc at a nearby level (I forget exactly, maybe T8-9), with Left sided NF stenosis. Excellent ILTESI PM Left from a tech perspective but minimal relief. All typical meds pursued prior. No h/o shingles, though zoster sin herpe considered.

Did a bone scan and interestingly, a CV jt at about T10 lights up like crazy, also a bit at nearby facets. So I do T9-10, T10-11 facets L, also T10 CV joint. Pain free 5 min later, we'll see how she feels in 4-6 weeks.

btw, for you EMG-philes out there, T PS EMG negative

h/o 'fibro", but this is not a whack a mole type pain, it stays in the same spot at each eval.

I should also add that, to my surprise, that CV jt at T10 was clearly ++DJD on fluoro image with big osteopyhtes. I landed on the rib just above the jt and walked down and off into it, was actually much easier than the facets. (she is osteopenic/obese so hard to even see pedicles clearly, for the facet lines)
