The NRMP website has a pretty good tutorial on how to do it. You can couples match between any two fields--just has to be the same match year. Depending on how you structure the list, you can end up at the same program or at different programs in the city. My wife and I were interviewing for the same specialty, so we applied to multiple programs in big cities. When a program that was the only program in the city offered me an interview but not her (as in, if I matched there, she would have no place to go in that city), I cancelled the interview.

I thought it was confusing at first, but it's in fact pretty simple. If you're both applying for competitive things or if one of you are, it's important to apply to LOTS of programs. Then when you finish your interviews and make your lists, you compromise and end up in programs that you both like and at least in the same city. Basically, you're not going to match into anything that is not on your rank list, so if you don't want to be in separate cities, don't rank different cities at the same number on the ROL.

It's all very doable and most programs in IM that we interviewed at were very supportive and enthusiastic--I can't speak for other specialties. Hope this helps!
