Cpt 64640 vs 64620

Hey guys,
Had some confusion with my coder wanted to clarify

Is it correct that essentially all our peripheral nerve rfa should be 64640 codes (genicular, hip, suprascapular, sacral lateral branch, etc)

And 64620 is applied for intercostal RFA? Or can we apply 64620 to more than just intercostal rf?

Wasn’t sure if this is correct but what I was told...thanks!

Yeah, 64620 is specifically intercostal while 64640 is other peripheral nerve or branch

do you do full thermals on the intercostals or do you do pulsed?

I've heard of people doing pulsed for actual treatment, but then thermal for completeness/billing. Haven't done this myself.

For sacral lateral branches 64450 and 64640, are you billing 77002 or 77003 for fluoro?
