People withdraw from programs before they start. The spots are difficult to fill because all of the internal candidates have jobs lined up ($$$). These spots sometimes get filled by international people who are more flexible. An open spot can cause a lot of problems with scheduling.
Call around very widely, you can probably find a spot somewhere, you might be suprised who has a vacancy right around now.
