CV, ERAS and including multiple universities

I'm a non-trad and have attending many universities during my career.

For my CV and ERAS, should I only list the universities where I obtained a degree?

To put a finer point on it:
1. For undergrad, I went to one university, then transferred to another where I received my degree. Should I list both or just the second?

2. I went to 3 different universities for my Med School pre-reqs. List all or none? No degree was obtained. I was basically just taking Ochem, Chem, Physics, etc.

3. After my undergrad, I went to graduate school, but did not finish. It was not medically related, and I ended up taking my career in a whole new direction. Leave out completely? Only reason I can think to include is that it explains why I moved across country from where I did undergrad/grew up.

Thanks for any advice.
 
I'd list them all - doesn't matter as much as AMCAS, but still worth listing, if only because random interviewers will ask you about it. I went to 1 school, spent a year abroad at another, took ochem over the summer at another - listed all 3 and ended up talking about them all on interviews. The place I took ochem at actually flagged my app as being an "alum" of their institution, which may have ensured it was seen by the PD, which was a little odd but kind of nice.
 
There are some licensing boards that want transcripts from every school you went to ever. Better to list them all now, so the place you match at has some idea how many places you need to get stuff from.
