Case/UH is more research oriented. This program aspires to be a typical academic medical center IM program. Metrohealth is the county hospital. Many more IMGs in the resident pool. Fellowship match list at UH is more heavily weighted to 'prestige' programs.

It's been a few years since I interviewed, but I can definitely say that if you are looking for a major U. hospital program, you definitely want the main program at Case Western. As mentioned above, Metrohealth is more of a county hospital type place, and not as academic. I have heard that the program there isn't as strong.
