Cystitis and cast

FA 2007 pg. 399 says acute cystitis result in WBC cast. but BRS path 4th edition Pg 263 indicate cystitis cause hematuria [RBC cast], but not WBC cast... BRS makes more sense to me, but does that mean FA is wrong? thanks!
 
i don't have the book right here....but w/o renal involvement (ie just cystitis) theres no way you have a cast of any kind....you sure they didn't just write "wbc's" for cystitis and not wbc casts (which = pyelo)

also....the rbc thing w/ the cystitis is also only rbc's.....you won't get an rbc cast w/o renal involvement (ie any nephritic syndrome)

Definitely agree with this poster... this is a crucial point. You can only ever have casts if the problem is in the kidney... if the problem is in the bladder (as in cystitis), you will just have the cells (in this case, WBC's for sure, and RBC's to a lesser degree).
 
The whole renal casts concept is confusing me too. Anyone have a simple explanation to remember what is going on?

the easiest way (imo) to think about it is this....a cast is basically a model of whatever is going on in the kidney....wbc's in the kidney->wbc cast, nephritic syndrome w/ rbc's and inflammation in the kidneys->rbc cast, dropping a ton of lipid through the kidney in nephrotic->fatty cast, etc, etc

if you only have cystitis (and not pyelo)...there is no problem in the kidneys-->no casts
