Unless you were supervisor of all tour guides (leader of the tour leaders), you weren't in a leadership postiion. Call it paid, non-clinical employment.

Thank you for your reply! This makes me question another one of my activities...

I was in a student organization where each member acted as a mentor for an incoming pre-med freshman and helped them with scheduling, applying to research, etc.. Since I did not have a leadership position in the club, I would consider this an extracurricular, not a leadership position, correct? Despite my title being a mentor.

I'd call that "mentor" and tag it "volunteer, non-clinical".

For some reason I never considered that employment could be listed under leadership! For my gap year, I’m working as a coordinator of one of the programs at a nonprofit (like actually running and organizing the program). Is it better to list this as leadership rather than employment?

Far better to call it employment. The leadership portion will be understood from your description. If someone is specifically looking to see if you are employed during your gap year, it is much better to make that very clear and easy to find.
