Between applying for fellowship and now coming close to ranking my husbands career situation changed and financially doing a fellowship a year from now would be way better for our family. Would it be possible to contact my top three and ask to defer my application for one year?
 
I think you have to withdraw from the match and apply again next year if rank order isn’t submitted yet. I would contact your top choices with a brief explanation of what happened, let them know you are still very interested and hope they will consider your application when you apply again next cycle.

Yeah it’s possible for sure. They might be sympathetic and understanding or not, hard to predict. If it is a very competitive fellowship consider that this might work against you for future applications.
 
You should withdraw from the match and contact the places you’re interested in going to explain that you will be reapplying next year and still are very much interested in pursuing the fellowship, but circumstances beyond your control don’t allow you to continue this year.
Having said that, if it was me, I would find a way to make it happen next year. You’ve got the momentum, current evals, etc. now.
