This is tough! I think your UCSF list is very convincing (particularly Language use and community engagement opportunities, experiencing something new - I personally moved coasts for med school which led to a brief LDR (my partner found a way to move to me) and loved it and highly recommend, it was worth it)
UCSF's prestige is very well known in Medicine, I wouldn't worry about that. Laypeople don't know about it mainly because it doesn't have an undergrad. You can tell people it's basically the med school of Berkeley and maybe they'll get it 🙂
UCSF is one of the big four IM residencies and you will definitely have amazing connections to match back east if that's what you want.
I'd say I'm slightly on the side of a vote for UCSF here - but you can't go wrong, Hopkins would be an amazing experience too. Can you visit both?
