Rad Onc requires a preliminary year of general medicine before your Rad Onc residency. This year could qualify for your first year of IM residency which is a Pre-requisite for Hem/Onc fellowship. No way to bypass internal medicine residency to Heme/Onc fellowship. If there were, we would have all taken it.
 
Possible to get credit for intern year? Some rad onc years?
Your subject line and message body are asking 2 different questions.

To answer the subject line: super easy. If you were competitive for Rad Onc you can totally get a good IM position for the last 2 (or maybe all 3) years and will absolutely match in a Hem/Onc fellowship.

To answer the message body: Maybe. Nope. In that order. Assume you've got another 6 years ahead of you and be blissfully excited if it only takes you 5.
