I'm pretty sure they don't exist. If they do, they're usually promised to the poor lab rat who's been slaving away in the PD's lab.

A couple of years ago, there were actually scramble positions open at 2 very prestigious residency programs (rumor had it that their interviews were very confrontational which may have turned off the top-flight interviewees they had)

The positions were eventually given to research fellows or via connections (IM resident at the same hospital system who switched in dermatology). I would be very surprised at an actual scramble for a dermatology spot.
 
A few years back there was this story about this derm applicant from USC who didn't match derm but scrambled into the intergrated plastics spot at UCLA that didn't fill.
 
There are currently at least two spots open for July 2009 on the East Coast. I can't be more specific than that; otherwise, I compromise my anonymity.

From what I understand, keeping close ties with your home department's PD - meaning, also, that you've been assiduously kissing ass since you arrived on campus - is one of the best ways to be in the know about off-cycle openings, as the PDs will know where they are. They'll provide the behind-the-scenes recommendations and tips that will push an applicant forward for these rare vacancies.
